Tirzepatide Price by Dose

The label schedule, and which doses actually matter

Tirzepatide titrates from 2.5 mg through 5, 7.5, 10 and 12.5 to 15 mg, escalating about every four weeks if tolerated. The first step is preparatory: 2.5 mg is a tolerance-building dose and is not intended to produce weight loss. The first therapeutic dose is 5 mg.

In SURMOUNT-1, mean reduction was about 15.0% at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg on the treatment-regimen estimand, against 3.1% with placebo. The effect rises with dose, which is why most patients escalate and why maintenance doses are where the money is actually spent.

Two pricing structures

Dose-tiered. The price rises with the dose. Brand works this way — Zepbound at $299, $399 and $449 direct from Lilly, with the rate contingent on refilling within 45 days. Several compounded programmes also escalate; one in our dataset rises from $245 on a twelve-month plan and explicitly increases at higher doses.

Flat-rate. One price across covered doses. NexLife (opens nexlife.us in a new tab) publishes flat pricing at every covered dose for its programmes, verified on its own plan pages. Oak Longevity, TrimRx, Mochi Health, Eden and Enhance.MD are each reported to hold price across doses, with different structures around membership.

Why this decides who is actually cheapest

A dose-tiered programme looks cheapest at signup and costs most at maintenance. A flat-rate programme looks less attractive at signup and does not move. Since you will not know your maintenance dose at signup, and since the evidence suggests treatment continues indefinitely, the maintenance price is the one worth comparing.

Worked against published figures: brand Zepbound at 2.5 mg for one month then 10 mg for eleven costs $5,088 over a year. A flat-rate compounded plan at $186 costs $2,232. That gap is entirely a function of dose structure, not of the headline monthly figure either provider advertises.

Watch for dose caps as well as escalation

Some programmes cap the maximum dose rather than charging more for it. One compounded semaglutide programme in our dataset is capped at 0.6 mg against the 2.4 mg used in the STEP trials, at $199 a month; the full-titration version is $279.

A capped programme is not a cheaper version of the same treatment. It is a lower-dose treatment with a correspondingly smaller expected effect, and the two should never appear in the same price ranking.

The question to ask

One sentence, in writing: what will I pay per month at 10 mg, including every required fee, and does it change again at 12.5 or 15 mg?
