The questions that test the claim

1. Flat across which doses, specifically? "Every covered dose" and "every dose" are different claims if the programme caps the maximum.
2. Does the flat rate survive a change of formulation, for example moving from microdose to standard?
3. Is it flat for the whole plan term, or flat until renewal?
4. What happens if the pharmacy or supply route changes?

The last one matters more than usual right now. The FDA has proposed excluding tirzepatide from the 503B bulks list, with comments closing 30 July 2026, and any compounded programme's pricing rests on a supply route that is under active regulatory review.
