The six charges that sit outside a headline price
Membership or programme fee. Sometimes monthly, sometimes annual. In our dataset these run from $19.99 to $149 a month. At the top of that range the membership can equal or exceed the medication price.
Shipping. Often free above a plan threshold and charged below it.
Initial consultation. Sometimes waived on enrolment, sometimes billed separately at over $100, and sometimes charged again if you lapse and return.
Laboratory work. Required by some programmes, optional in others, and billed variously to you or to insurance. This is the field we most often cannot establish from a published page.
Dose surcharge. The largest and least visible. Brand products move $150 between starter and top tier; several compounded programmes escalate too.
Cancellation and restocking terms. Not a monthly cost, but a real one if you stop early on a prepaid plan.
The one question that surfaces all of it
Ask, in writing: what is the total I will pay over twelve months if I titrate to a maintenance dose, including every required fee? A provider who answers in one sentence is being straight with you. A provider who redirects to the monthly figure has told you something too.
