Why annual totals reverse monthly rankings
Two things only become visible over twelve months: dose escalation and prepayment conditions. A programme that looks $50 cheaper in month one can be $2,000 more expensive by month twelve, entirely because of dose structure.
Worked against published rates: brand Zepbound following the label schedule costs roughly $5,088 across a year, because it moves from $299 to $399 to $449 as the dose rises. A flat-rate compounded plan at $186 is $2,232 across the same year. Neither headline monthly figure predicts that gap.
The prepayment question
Most of the lowest annual figures in this market require twelve months paid upfront. That lowers the monthly equivalent by 10 to 20 per cent and commits the full amount before you know whether you tolerate the medication, respond to it, or stay on it.
Three answers are worth having in writing before prepaying: what is refunded if you stop for a clinical reason, what happens if supply is interrupted for regulatory reasons, and whether the rate holds if your dose or formulation changes.
