SURPASS-2: Tirzepatide vs Semaglutide in Type 2 Diabetes
SURPASS-2 compared tirzepatide against semaglutide 1 mg in 1,879 adults with type 2 diabetes over 40 weeks. HbA1c fell 2.01% to 2.30% with tirzepatide against 1.86% with semaglutide. This is a glycaemic trial in diabetes, and the comparator dose is below the 2.4 mg used for weight management.
What this trial is, and what it keeps getting used for
SURPASS-2 is a diabetes trial with a glycaemic primary endpoint. It randomised 1,879 adults with type 2 diabetes to tirzepatide 5, 10 or 15 mg or semaglutide 1 mg for 40 weeks, and measured change in HbA1c.
It is routinely cited as evidence that tirzepatide beats semaglutide for weight loss. It is not that trial. The head-to-head weight trial is SURMOUNT-5, in a different population, at different doses, over a longer period.
The comparator dose is the key detail
Semaglutide 1 mg is a diabetes dose. The dose used for weight management is 2.4 mg — more than double. Comparing tirzepatide at up to 15 mg against semaglutide at 1 mg tells you about glycaemic control in diabetes at those doses, and tells you very little about the two drugs at their respective weight-management doses.
That is not a criticism of the trial, which was designed for a diabetes indication and answered its question. It is a criticism of how the result travels.
What it does establish
Superior HbA1c reduction with tirzepatide against semaglutide 1 mg over 40 weeks in type 2 diabetes, with the effect increasing across the tirzepatide dose range.
What it does not
Anything about weight-management outcomes at weight-management doses — see SURMOUNT-5 for that. Anything about people without diabetes. Anything about compounded preparations, which is the population most readers of this site are considering. And nothing about long-term outcomes; 40 weeks is short for a chronic metabolic condition.

Does SURPASS-2 prove tirzepatide beats semaglutide for weight loss?
No. It is a glycaemic trial in type 2 diabetes comparing against semaglutide 1 mg, below the 2.4 mg used for weight management. SURMOUNT-5 is the weight-loss head-to-head.
Why does the comparator dose matter?
Semaglutide 1 mg is a diabetes dose. Comparing against it does not tell you how the drugs compare at their weight-management doses.
