FDA Approves the Wegovy Pill: Oral Semaglutide 25 mg for Weight Management

The approval
On 22 December 2025 the FDA approved the Wegovy pill — once-daily oral semaglutide 25 mg — to reduce excess body weight and maintain reduction long term, and to reduce the risk of major adverse cardiovascular events. It launched in the US in early January 2026.
The approval rested on the OASIS trial programme and on SELECT, the cardiovascular outcomes trial for injectable semaglutide. The dual indication matters: this is the only GLP-1 indicated for weight loss that also carries a cardiovascular event-reduction indication for adults with established cardiovascular disease and overweight or obesity.
OASIS 4, in detail
A 64-week phase 3 randomised, double-blind, placebo-controlled trial in 307 adults with obesity, or overweight with at least one weight-related comorbidity, excluding people with diabetes. Mean baseline weight 106 kg, mean BMI 38. Participants were randomised 2:1 — 205 to oral semaglutide, 102 to placebo — with a 12-week dose escalation, 64 weeks of treatment and a 7-week off-treatment follow-up.
Two figures are published and they differ meaningfully. On the treatment-policy estimand, which includes all participants regardless of adherence: 13.6% mean reduction against 2.2% with placebo. Among those who remained on treatment: 16.6% against 2.7%. One in three adherent participants achieved 20% or greater reduction, against under 3% on placebo.
Marketing tends to quote the higher figure. Both are legitimate; the analysis behind each should be stated, and frequently is not.
The safety finding that stands out
Serious adverse events occurred less frequently with oral semaglutide than with placebo — 3.9% against 8.8%. That is unusual enough to note, and it reflects that the placebo arm in an obesity trial is not a healthy population.
Post hoc analyses showed improvements in glycaemic control, blood pressure and cardiovascular risk factors, with normalisation of blood glucose in more than 70% of participants who had prediabetes at baseline.
The practical constraint
Oral semaglutide is a peptide, and peptides absorb poorly by mouth. The formulation works, but it requires administration in the morning on an empty stomach, with no more than four ounces of water, waiting 30 minutes before eating or drinking anything else.
That is a real adherence burden and it is the clearest practical difference from orforglipron, which has no such restrictions. For some patients it will be decisive; for others it is a minor daily routine.
Pricing and uptake
The starting dose launched at $149 a month with savings offers. Novo Nordisk reported the pill surpassing three million prescriptions in just over five months from launch — a rate that suggests the oral route addressed a genuine barrier rather than merely offering an alternative.
It is covered under the Medicare GLP-1 Bridge at $50 a month for eligible Part D enrollees from 1 July 2026.
Where it sits
On mean reduction, oral semaglutide sits between injectable semaglutide and tirzepatide depending on which estimand you use — 16.6% adherent is above the 13.7% injectable semaglutide recorded in SURMOUNT-5, which is counterintuitive and reflects different trials, populations and durations rather than a demonstrated superiority of the oral route.
Against compounded semaglutide, which runs $110 to $299 a month in our dataset, the approved oral at $149 represents a genuine competitive shift: an approved product with published data at a price inside the compounded band.
