What began on 1 July
The Medicare GLP-1 Bridge, run by the Centers for Medicare & Medicaid Services, gives eligible Part D enrollees access to certain GLP-1 medications for a flat $50 monthly copayment. It covers Wegovy, Zepbound, the Wegovy pill and Foundayo.
This is a demonstration programme rather than ordinary Part D coverage, and the distinction is structural. Federal law excludes drugs used for weight loss from Part D. The Bridge operates outside those rules: a central Medicare system handles approvals, processes claims and pays pharmacies. Your Part D plan does not have to participate and is not the route.
The flattening effect
All four products cost the same $50. That collapses a self-pay spread running from $149 to $449 into a single figure, and it changes the nature of the decision entirely.
For an eligible patient the question stops being which product is affordable and becomes which is most effective and best tolerated. On mean reduction that ordering is tirzepatide at about 20.9% in SURMOUNT-1, oral semaglutide at 13.6 to 16.6% in OASIS 4, injectable semaglutide at about 13.7% in SURMOUNT-5, and orforglipron at roughly 11 to 12.4% in ATTAIN-1.
It is the first time in this category that a patient has been able to choose on clinical grounds alone.
The expiry, and what sits behind it
The Bridge runs to 31 December 2027. CMS originally indicated it would end in December 2026, with the BALANCE Model — intended to lower prices through direct negotiation with manufacturers — starting in January 2027.
In May 2026 CMS announced that the Part D portion of BALANCE has been delayed indefinitely, and extended the Bridge instead. So the current position is eighteen months of access with no confirmed programme behind it.
That matters clinically as well as financially. SURMOUNT-4 found that participants withdrawn from tirzepatide regained 14% of body weight over the following year while those continuing lost a further 5.5%. Starting treatment under a programme with a known end date, and no announced successor, is a decision worth making with that evidence in view.
What it does to the compounded market
For eligible patients it largely ends the argument. $50 for an approved product with premarket review, published trial data and manufacturer supply is below every compounded programme in our pricing dataset — the cheapest of which is $110 a month for a microdose sitting below any studied dose.
It also arrives while the FDA proposes closing the last pathway for large-scale compounding of these molecules, with that comment period ending 30 July. Access expanding at the approved end while it contracts at the compounded end is not a coincidence of timing so much as a policy direction.
For the much larger group not eligible — most people under 65 without commercial coverage — nothing changes. Self-pay remains the market, and the comparison is manufacturer-direct against published flat-rate compounded pricing from $186 a month.
What to establish if you think you qualify
- Confirm eligibility through Medicare directly rather than through a telehealth platform.
- Ask your prescriber which of the four products they recommend and why — the financial variable has been removed.
- Ask what happens after December 2027. Nobody knows, and a clinician who says so is being straight.
- If you currently pay cash for compounded medication, re-run the comparison. $50 against $110 to $399 is not close.
