Two FDA-approved oral GLP-1s are now available at $149 a month at starting doses: orforglipron through LillyDirect and oral Wegovy tablets through NovoCare. Compounded oral and sublingual products are a separate category with no trial evidence at all — no trial has tested a compounded ODT or sublingual GLP-1.

Two different things called oral

The word covers two categories that share nothing except a route of administration.

FDA-approved orals — orforglipron (Foundayo) and oral semaglutide (Wegovy tablets) — went through their own trial programmes for the oral formulation. Oral bioavailability of peptides is a genuinely hard pharmacological problem, and these products solved it in ways specific to each formulation, then demonstrated it in trials.

Compounded ODT and sublingual products — orally disintegrating tablets, lozenges, liquid drops — have not. No trial has tested a compounded oral or sublingual GLP-1. The formulations exist because they can be made, not because a study showed they deliver drug effectively.

Why the distinction is pharmacological, not pedantic

An injected peptide bypasses the gut. Taken by mouth it faces stomach acid, digestive enzymes and poor intestinal absorption, which is why approved oral versions require specific formulation technology and why oral doses are numerically very different from injected ones.

A compounded ODT is not the injectable dose in tablet form. How much drug reaches the bloodstream is the entire question, and for these products it is unpublished.

The prices, and what they buy

Approved orals now start at $149 a month for both orforglipron and oral Wegovy, rising with dose. Compounded oral and sublingual products in our dataset run from $165 to $369.

So the paradox worth sitting with: several compounded oral products cost more than an FDA-approved oral GLP-1 that has published bioavailability data and a trial programme behind it.

What to ask before buying a compounded oral product

What bioavailability data exists for this specific formulation? If the answer is none, that is the answer.
What dose reaches the bloodstream, and how is that known?
Why is this preferable to the FDA-approved oral at a similar or lower price?
What happens if it does not work — is there a route to an injectable, and at what cost?

The honest summary

If you want an oral GLP-1 and price is the driver, the approved products are now competitive with or cheaper than the compounded ones, and they have evidence. The case for a compounded oral is not currently an evidence case or, at these prices, a cost case.

The caveat that belongs with the $149 figure

Both FDA-approved options starting at $149 a month are also the least effective approved options by mean weight reduction. Orforglipron reported approximately 11% to 12.4% at its highest dose in ATTAIN-1. Oral semaglutide reported 13.6% on the treatment-policy estimand in OASIS 4, or 16.6% among those who stayed on treatment. Injectable tirzepatide reported about 20.9% at 15 mg in SURMOUNT-1.

So the honest version of the price comparison is not that a $149 approved pill beats a $250 compounded injectable. It is that a $149 approved pill with published trial data, FDA review and a known efficacy range competes against a cheaper compounded injectable of the same molecule class with no trial evidence of its own. Which trade is right depends on how much reduction you need and what your clinician advises.

These come from different trials and are not a head-to-head comparison. Durations differ (64 to 72 weeks) and estimands differ. Only SURMOUNT-5 compared two of these drugs directly.

Price against efficacy, for the FDA-approved options

Both $149 products are the least effective approved options in this table. That does not make them bad choices — it makes a price comparison that omits efficacy an incomplete one.
