Why this list needs a warning at the top
A list ordered by price alone puts microdose programmes first, because they cost least. They also deliver roughly 1 mg a week against the 5, 10 and 15 mg studied in SURMOUNT-1, where mean reduction rose from about 15.0% to 20.9% across that range.
So the cheapest entries here are not cheaper versions of the same treatment. They are a different, lower dose with no trial evidence of its own. We label the program type on every row, and we never rank a microdose against a standard programme as though they were comparable.
What genuinely sits under $200
Standard compounded injectables: a handful, mostly on twelve-month prepaid plans. Those rates require paying the full term upfront, and the same providers' month-to-month rates are considerably higher — one charges $289 against a $169 prepaid figure, a 71% premium.
Approved orals: both start at $149 at their starting doses. Those are FDA-approved products with published trial data, which no compounded programme has.
Microdose programmes: from $110. Below any studied dose.
The two routes that beat everything on this page

What a sub-$200 price usually costs you
- Flexibility. Most require twelve months paid upfront.
- Dose headroom. Some cap the maximum dose; one compounded semaglutide programme is capped at 0.6 mg against the 2.4 mg used in the STEP trials.
- Clinical contact. All-inclusive cheap programmes generally include less clinician time than membership models charging more.
None of those is hidden — they are in the terms. They are simply not in the headline.
